POSITION SUMMARY:
Under the Supervision of the Director of Integrated Health, the Home Visitor is responsible for delivering the Parents as Teachers (PAT) home visiting model to families with children from prenatal to age 5. This role involves partnering with parents to support their child's development and learning through regular home visits, developmental screenings, and connecting families to community resources. The Home Visitor will work to enhance parent-child interactions, promote development-centered parenting, and improve family well-being.
ESSENTIAL DUTIES AND RESPONSIBILITES:
The duties below are intended to provide an overview of the duties required of the Home Visitor.
- Home Visits: Conduct regular, scheduled home visits to provide personalized support and education to families. Use the PAT Foundational Curriculum to guide activities and discussions.
- Parent-Child Interaction: Promote positive parent-child interactions through activities and guidance that support the child's development.
- Developmental Screenings: Conduct developmental screenings using tools such as the Ages and Stages Questionnaires (ASQ-3) and Ages and Stages Questionnaires: Social-Emotional (ASQ:SE-2). Identify any developmental concerns and make appropriate referrals.
- Family Well-Being: Support families in setting and achieving goals related to their child's development, health, and overall family well-being. Connect families to community resources and services as needed.
- Group Connections: Facilitate group connections to provide families with opportunities to build social networks and learn from each other.
