This program provides behavioral health services to adults ages 18-59 with severe and persistent mental health diagnoses.

This position will serve as a case manager working with adults primarily involved with Full Service Partnership services – including but not limited to: intensive case management services with a small caseload of individuals in need of FSP level services; team-based care coordination with both clinic and community service providers; extensive field work; work with client population at high risk of hospitalization, homelessness, and/or incarceration.

The most competitive candidates will be bilingual, have field work experience; experience working with individuals who have severe and persistent mental health diagnoses; and team based experience.

The schedule for this position is a 9/80 schedule with every other Friday off.

A valid CADL is required.

Under supervision, the Incumbent will team with mental health professionals in planning and providing mental health services; provide general counseling, case management, and community services and to do other work as required.

Incumbents in this class team with behavioral health professionals in the provision of consumer treatment, to directly assist clients in the utilization of appropriate community resources, and to provide education and information to the community.

Incumbents in this class provide support, encouragement, advocacy, and related services in order to assist the client in coping with immediate situations with no attempt to modify or change the consumer's personality structure. Classes in this series differ from those in the Clinical Therapist series in that the latter, due to advanced education and experience, use independent judgment in making diagnoses, developing treatment plans, and providing a wide range and variety of behavioral health services including psychotherapy.
