Description & Requirements
In this role, there is no direct hands-on care. We assess individuals in homes, facilities, and group homes for Long Term Care needs and facility placement.
*Must be able to commute into and cover one of the following counties within the state of Maine: Androscoggin, Kennebec, Cumberland, York or Somerset County
- Knowledge and understanding of medical and/or behavioral health diagnoses and prescribed medications
- Knowledge of community support programs, long-term care assessment and level of care in medical, behavioral health or related programs, preferred
- Ability to collect data, define problems, establish facts, and draw valid conclusions
- Excellent written and verbal communication skills
- Excellent interpersonal and customer service skills
- Proficient in Microsoft Office Suite
- Excellent organizational skills and attention to detail
- Ability to work in a fast-paced environment
- Ability to work independently and meet timelines
Education and Experience Requirements:
- RN or LPN License in the state of Maine or hold a compact RN or LPN license
- One (1) year of clinical experience
- Experience in long term care or home health care preferred
- Regional travel is required to perform in-person assessments
Home Office Requirements:
Some work may be completed at home after field assessments have been completed.
- Maximus will provide laptop, printer, and cellphone
- Reliable high-speed internet service
- Minimum 20 Mpbs download speeds/50 Mpbs for shared internet connectivity
- Minimum 5 Mpbs upload speeds
Training and Work Schedule:
Weekends and holidays require rotation or on call availability
- Two weeks training via Zoom followed by in the field assessment training with 1:1 support
- Standard hours: 8am- 5pm Eastern, some flexibility available after training
Essential Duties and Responsibilities:
- Responsible for completing medical and/or behavioral health assessments within contract requirements
- Conducts on-site, video call and/or telephonic assessments to determine an individual’s needs for services and supports, eligibility, level of care or related outcome
- Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures.
- Travel may be required based on program contract requirements
- Performs other related duties as assigned.

- High School Degree or equivalent required
- Minimum 1 year of clinical experience required
- LPN, RN, LCSW, or related licensure preferred and may be required based on contract requirements
